If CHS provides an individual with access to PHI, the following will be met:

1. CHS must provide the PHI in the form or format requested if it can be produced in such manner.  If not, a readable hard copy as agreed upon by both parties.
2. CHS may provide the individual with a summary of the PHI requested rather than access to the PHI, or an explanation of the PHI if:
   • The individual agrees in advance; and
   • The individual agrees to any fees for such summary

CHS must, in accordance with the time limits described, provide a convenient time and place for the individual to inspect or obtain a copy of the PHI. In general, this will be normal business hours of CHS. Use of private offices or conference areas is acceptable.  Other locations and times must be approved by the Privacy Officer.

If the individual requests copies of PHI or agrees to a summary or explanation of the PHI, CHS may impose a charge based on established policies.  Such fees shall only be imposed for copying, postage and the preparation of explanations and summaries if agreed to by the individual.
