CHS must document and retain the following:

1. The information required to be included in an accounting;
2. A copy of the written accounting that is provided to the individual; and
3. The title of person or office responsible for receiving such requests.

Copies of accountings will be retained within the medical record of the individual. The CHS Compliance Office is responsible for receiving requests for accounting.
