CHS will permit an individual to request that CHS amend the PHI maintained in the designated record set. CHS requires such requests to be in writing, either using the CHS Request for Amendment form, or in a manner that addresses the information required on the CHS Request for Amendment form. Such requests must provide a reason to support the requested amendment.

CHS will act on the individual�s request for amendment within 60 days of receipt. Such actions will be:

1. Grant the amendment in part or whole;
2. Deny the request in whole or part, and provide the individual with a written denial.

If CHS is unable to act within 60 days, CHS may extend the time for such action by no more than 30 days provided that:

1. CHS provides the individual with a written statement of the reasons for the delay and the date by which it is expected CHS will complete its actions; and
2. CHS may have only one such extension per request for amendment.
