If CHS accepts a requested amendment, in whole or part, the following requirements will be met:

1. CHS will make the amendment to the PHI that was the subject of the amendment by identifying the records in the designated record set that are affected by the amendment and appending or otherwise creating a link to the amendment.
2. CHS will inform the individual that the amendment is accepted and obtain the individual’s identification of and agreement to have CHS notify all parties  with which the amendment needs to be shared.
3. CHS will make every reasonable effort to inform and provide the amendment within a reasonable time to:
   • persons identified by the individual as having received PHI and needing the amendment; and
   • persons and business associates of CHS that CHS knows have the PHI that is subject of the amendment and that may have relied or could rely on such information to the detriment of the individual.
