CHS will make the Notice of Privacy Practices, (NPP), available on request to any person upon request.
The provision shall take place no later than the first time the patient presents for treatment at CHS.
In an emergency situation, CHS will make every effort to provide the NPP as soon as practicable.
Except in emergency situations, CHS will make a good faith effort to obtain a written acknowledgment of receipt of the notice. This written acknowledgment will require the patient to sign the specified area of the NPP form. If such acknowledgment is not obtained, CHS will document on the NPP form the reasons why the acknowledgment was not obtained. The patient maintains eligibility for services regardless of the compliance with the acknowledgment statement.
The NPP will be posted within the Information boards of CHS and copies will be available for others upon request.
At such time as the NPP is revised, CHS will make the NPP available upon request on or after the effective date.
CHS will maintain an electronic version of the NPP on the CHS web site. A downloadable version will be available on the web site.
If a patient requests to receive the NPP by email, CHS will make every effort to comply based upon the accuracy of the requestor’s address. If CHS becomes aware of a delivery failure, note will be made and a paper version will be made available to the patient. The individual who requests an electronic version of the NPP retains the right to a hard copy of the NPP.
